Starting April 1, 2024, all therapeutic patients will be accommodated at OneBlood Donor Centers. Your well-being remains our top priority, and will continue providing the care you deserve at OneBlood’s Local Donor Centers.
What You Need to Know:
Transition to OneBlood:
- All therapeutic patients can now be seen at OneBlood Donor Centers.
- To facilitate this transition, please complete the prescription order form (Form 96). Your referring physician must fill out the form, ensuring a seamless continuation of your treatment.

Kindly ask your referring physician to complete the prescription order form at their earliest convenience.
